Submit a privacy request

Use this form to exercise your privacy rights under GDPR Articles 12–22, UK GDPR, Swiss FADP-equivalent rights, US consumer health data laws such as Washington's My Health My Data Act, Canadian privacy law such as PIPEDA and Quebec Law 25, Australia's Privacy Act, and New Zealand's Privacy Act, Brazil's LGPD, and Mexico's LFPDPPP/ARCO rights, plus applicable privacy rights in Japan, South Korea, Singapore, Hong Kong, Taiwan, Israel, Chile, Colombia, Argentina, Peru, and South Africa. Eutopix Labs P.S.A., as the controller for Forkin, will reply within the applicable legal deadline: normally 30 days for GDPR/UK GDPR/Swiss/Canadian requests, 45 days for US consumer health data requests, and the period required by Australian, New Zealand, Brazilian, Mexican, Asian priority-market, Latin American, Israeli, or South African law where those regimes apply.

Requests are free of charge for the first one. We may ask you to verify your identity (typically by replying from the email associated with your account) before we act, to prevent someone else from exercising your rights on your behalf.
